Salary: $52,794.72 - $71,152.32 Annually
Lead field-based outreach efforts that support Los Angeles County's homelessness response. This role oversees Community Health Workers in the field, coordinates crisis response, supports encampment operations, and helps ensure timely service connections for vulnerable populations.

What You'll Do:
- Provide direct field supervision, coaching, and support to outreach staff and senior staff
- Coordinate urgent field response involving medical, behavioral health, public safety, and service needs
- Support encampment resolution planning, pre-engagement activities, and follow-up field operations
- Oversee HMIS documentation, By-Name Lists, data quality, reporting, and compliance with program requirements
Minimum Requirements:
- Option I: Three years of experience at the level of Senior Community Health Worker, or similar experience in a public or private community-based organization, clinic, hospital, or health system
- Option II: Five years of experience at the level of Community Health Worker in a public or private community-based organization, clinic, hospital, public health organization, or health system
- Valid California Class C Driver License, or ability to use an alternative method of transportation
Preferred Experience:
- Recent experience with a public homelessness services agency providing direct field-based services, such as LAHSA
