CCBHC Care Coordinator

Seasons is a comprehensive Behavioral Health Center offering a broad range of psychiatric and behavioral health services to communities in Northwest Iowa. We are currently seeking a highly organized and passionate individual to be our CCBHC Care Coordinator.

The CCBHC Care Coordinator will:
·        Coordinate with both internal and external care providers to ensure that clients are connected with the services they need
  • Work as part of an interdisciplinary care team to ensure clients receive the highest level of care and that project goals and objectives are obtained
  • Facilitate day to day collaboration and communication on the clients’ behalf across multiple levels of care
  • Understand and monitor client’s health and wellness needs and identify interventions to address gaps in service
  • Provide education and training to clients and their families in order to promote health and assist in the understanding of mental illness, medication compliance, and additional specialty services as recommended by medical providers
·        Assist with the collection, maintenance, and analysis of participant data, including the administration of assessments and evaluations
·        Participate in agency meetings and trainings, as well as health promotion and wellness activities

Requirements:
  • A Bachelor’s degree in Social Work, Human Services or a related field is required
  • An active Iowa license (BSW) is preferred
  • 3+ years of related experience is preferred
  • Experience administering assessments and completed data collection is preferred
  • Must have excellent communication and organizational skills
  • Some travel is required for this position, so a valid driver’s license is required
