Case Manager - Mental Health

Position Title:     Child Mental Health Case Manager  
Employment Classification:   Independent Contractor (1099) This is a 1099 position. Persons paid on a 1099 basis are independent contractors and are self-employed. Independent contractors are required to pay all self-employment taxes (Social Security & Medicare) as well as income tax. 

Responsible for the case management and mental health rehabilitation support of up to 50 Consumers. Works with children, adults and families to create and achieve treatment goals, quality of life and promote recovery.

Major Areas of Responsibility:
§ Works with children, adults, and families with behavioral, emotional and mental illnesses.
§ Works as a partner in a collaborative decision making team and independently, making significant case plan decisions regarding families assigned for targeted case management services.
§ Mental Health Rehabilitation Services include: linking, monitoring, collaborating, teaching, coaching, role modeling, and supporting families with mental illness in order to reduce impairments and maximize their growth that interfere with their daily functioning.
§ Provides support with housing stability, effective employment readiness, school completion, independent living skills and money management.
§ Addresses barriers and obstacles to help the families achieve permanence in the home, school, workforce and community.
§ Reduce recidivism for our delinquency cases.
§ Provides crisis management services.
§ Coordinates with all team members monthly to ensure a healthy collaborative approach. 
§ Follows and abides by Medicaid guidelines, including daily documentation and a 24-hour turn around in submitting progress notes. 
§ Participates in Full Service Treatment Team meetings monthly.
§ Work with consumers to develop and implement individual Care Plans; including identifying goals and milestones.
§ Link families with available programs and community resources. 
§ Follows Recipient Rights guidelines.
§ Participates in required staff meetings and training.
§ Attends court hearings and medical appointments with families, as needed.
