Location: Golden Valley, MN
Caseloads: MHP (Mental Health), Elderly, Intellectual Disabilities, BCBS, Medica, and CADI
Counties: Anoka County, Chisago County, Dakota County, Hennepin County, Ramsey County and Stearns County
Schedule: Monday - Friday (Full time)
Starting wage: $44,600.00 / Masters $46,600.00 with a $500.00 sign-on bonus after 3 months
MERIDIAN SERVICES has served people with intellectual disabilities, those with mental health issues, the elderly and their families for a quarter of a century. Currently our Case Management Department serves approximately 2,500 elderly, 1,500 people with mental health issues and more than 1,000 persons with intellectual disabilities.

Job Summary:
Our Case Managers perform all requirements of Rule 185 including: needs assessment; individual service plan development; screenings; coordinating team activities; assisting with planning for new service development; locating residential, vocational, and other needed services; monitoring service delivery; conducting consumer visits; developing waiver budgets; and acting as advocates for consumers.
