Currently searching for an RN or LPN interested in working in home care. Our professional health care staff is specially trained to provide patient-centered medical care in the warmth of the patient's home. By offering a wide range of services, we are capable of providing multiple levels of care that are based entirely on individual needs. As a Nurse Case Manager at Comfort Care, you work directly with the patient in their home, providing nursing services as well as case management services which include but are not limited to : coordinating care plans, communicating with physicians and other health care entities involved in the patient's care, and working with your team at Comfort Care to ensure clients are receiving all of the necessary care to keep them safe and healthy at home.
