LTRG Case Manager

SUMMARY: Responsible for providing case management to disaster survivors; includes in-depth assessments of their long-term recovery needs and assist them in accessing available funding. Responsible for supporting survivors to receive a full range of appropriate services in a planned, coordinated, efficient and effective manner. The work is performed under the general supervision of a supervisor; however, the employee is expected to exercise initiative, creativity and good judgment in carrying out duties. The employee must be available for flexible work hours.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Identify, screen, and enroll survivors for Case Management services.

Assess disaster recovery needs.

Assists the survivor in short-term planning and referrals for basic and immediate needs.

Assists the survivor in coordinating necessary services and resources to address disaster recovery needs.

Gather, record, and maintain detailed casework records and files maintaining confidentiality. 

Empowers the survivor to effectively access resources available.

Advocate for survivors receiving case management services.

Refer survivors to other services as appropriate.

Prioritize cases and set objectives in consultation with the long-term recovery committee.

Establishes and maintains communication with other agencies and staff involved with survivors.

Attend meetings and trainings.

Works as a member of a team to provide effective, quality services.

All other duties as assigned.
