School Based Case Manager
Description
Adult and Child Center partners with over 100 schools in central Indiana to help students with behavioral health needs. Our Case Managers work one-on-one with students in the classroom, or in a school setting, to build needed skills. They also work with students and their families/caregivers in the home to provide a full range of support.
Our Case Managers work with our School-Based Teams, which also include a Team Leader, Psychologist, and Staff Therapist. Case Managers help children and teens develop behavioral health-related skills to succeed academically and socially, creating a lasting impact throughout their lives.
Responsibilities include but are not limited to:
- One-on-one skill building with students
- Skill building with caregivers/families in the home setting
- Case management
- Crisis intervention/safety planning
- Create accurate, timely progress notes and reports/documentation
Requirements
- Bachelor's degree in Social Work or related behavioral health field
- Previous experience in behavioral health or working with children/teens preferred but not required
- Interested in helping students, grades pre-K through 12, with behavioral health needs
- Available during school hours, as well as after school to meet with students and caregivers
- Strong organizational skills
- Ability to maintain professional relationships with a variety of individuals (psychologists, teachers, guidance counselors, and other school/A&C staff)
- Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
