Social Worker/Case Manager

Position: Social Worker/Case Manager
  
Locations: The primary location for this position is in Golden Valley, MN or Hopkins, MN. Travel throughout the Twin Cities is required for meetings and visits with persons served. Based on caseload, travel throughout other parts of Minnesota may be required. 
 
Caseloads:  Elderly/AC, Intellectual Disabilities, Medica, BCBS, Hennepin Health, and CADI
 
Counties: Caseloads may include Anoka County, Chisago County/Anoka/Isanti, Dakota County, Hennepin County, and Ramsey County
 
Schedule:  Monday - Friday (Full time) Typical daytime hours with occasional evenings as needed for meetings
 
Starting wage: $48,600.00 Annually. We offer a higher starting salary for employees with a master’s degree.
$1000 sign-on bonus after 3 months

Meridian Services has served people with intellectual disabilities, those with mental health issues, the elderly and their families for over a quarter of a century. Currently our Case Management Department serves approximately 2,500 elderly, 1,500 people with mental health issues and more than 1,000 persons with intellectual disabilities.

Job Summary:
 As a Case Manager/Social Worker, you will be working on finding resources and services for persons (clients) that best fit their needs and situations. At Meridian Service’s, each Case Manager is an advocate for persons with disabilities/elderly and helps to make a difference in their lives. Our Case Managers will carry caseloads of approximately 60 persons with intellectual disabilities, mental health diagnosis and/or elderly persons. As a Case Manager you will be performing the requirements of Rule 185.
 
In this position you will be completing the following:
*Visits with persons on the caseload and attending team meetings
*Completing needs assessment; individual service plan development (CSSP)
*Attending and completing screenings
*Assisting with planning for new service development
*Locating residential, vocational, and other needed services depending on each person’s needs
*Monitoring service delivery and ensuring the health and safety needs of each person are being met.
*Completion of referrals and crisis intervention as needed
*Developing waiver budgets
*Completing case notes
*Acting as advocates for people we provide services for
 
 
 
Requirements:
*Be licensed as a Social Worker by the Minnesota Board of Social Work (Bachelor of Social Work required for licensure) or licensed as a Graduate Social Worker (Master’s in Social Work required for licensure) by the Minnesota Board of Social Work.
* A valid driver’s license and daily access to reliable transportation to travel throughout the Twin Cities and other parts of Minnesota based on caseload.
* Current vehicle insurance
* Have an acceptable driving record as determined by Meridian Services policy.
* Successful clearance from a DHS background Study
 
Nice to have:
*Experience providing case management services
*Experience working directly with individuals with intellectual disabilities, mental health diagnosis or elderly  persons.
* Knowledge of community resources and providers
* Excellent computer skills including Microsoft office
* Great organizational skills and attention to details.
* Knowledge of Medical Assistance, Medicare and MN Health care programs
