Community Health Worker

ORGANIZATION SUMMARY:
Pillsbury United Communities (PUC) works with underestimated populations across Minneapolis to foster the resilience and self-sufficiency of individuals, families and community as a whole. PUC designs solutions that address a spectrum of intersecting needs from education to employment to health, while continually investing in our own capacity to innovate and improve. PUC is creating a new model for nonprofit organizations with an interconnected system of programs, community centers, and social enterprises connects with over 55,000 individuals each year.
 
JOB SUMMARY:
The CHW will work closely with colleagues at Brian Coyle Center, across Pillsbury United Communities (PUC), and alongside partner organizations to improve community health outcomes. This person will focus its work on addressing social determinants of health and the elimination of health disparities.
 
The CHW will demonstrate strong skills in case management, health promotion and education, outreach and relationship-building, and partnership development. The CHW will also engage in broader systems change.
 
JOB RESPONSIBILITIES:
·   Convey program objectives, services, and anticipated impact to the user population.
·   Conduct outreach to mosques, churches, residential high-rises, adult day care centers, and other
community places to raise awareness and recruit participants for programming.
·    Coach clients in effective management of their chronic health conditions and self-care.
·   Build and maintain positive working relationship with clients, families, providers, partner agencies,
supervisor, and staff.
·   Complete required trainings and certifications including Chronic Disease Self-Management.
·   Develop strategies for engaging youth, women, and elders around health and wellness issues.
·   Develop necessary outreach and engagement materials to raise awareness about services . 
·   Develop collaborative partnerships with stakeholders to increase resources for participants and work
toward improving community health outcomes.
·  Implement and continuously assess the effectiveness of health promotion and education curricula
and other program activities.
·  Prepare and conduct program evaluation to measure the impact of programs and services offered.
·  Attend and contribute positively to all scheduled staff meetings.
·  Collect, maintain, and report data to appropriate partners in a timely manner.
·  Assist the supervisor in developing quarterly and annual reports to funders and other stakeholders.
·  Perform other duties as assigned by supervisor.
 
REQUIRED QUALIFICATIONS:
Required:
Associate Degree or higher in Public Health, Community Health, Human Services, Sociology, or
similar field and 2+ years of community-based experience.
Prior experience working with East African communities, particularly Somali and/or Oromo; and
knowledge of Cedar-Riverside community resources.
Fluency in English and Somali language(s).
Knowledge of Microsoft Word, Excel, Power Point, and Outlook.
 
Preferred:
• Certification as a Community Health Worker (CHW).
Ability to speak other East African language(s) and demonstrated cultural competence.
Possess valid Minnesota driver’s license.
Lived experience in Cedar-Riverside neighborhood and/or an East African country.
Professional experience in Cedar-Riverside neighborhood.
