Care Coordinator

Sandra Eskenazi Mental Health Center, Indiana’s first community mental health center has a philosophy of care that stresses strength-based, community centered treatment that utilizes the recovery model and welcomes all patients from children to seniors. 
 
Sandra Eskenazi Mental Health Center’s mission is to Advocate, Care, Teach and Serve, with special emphasis on the vulnerable populations of Marion County, Indiana. We are comprised of several clinics, outreach centers and community based services whose primary mission is to serve individuals with serious mental illness and chronic addictions, as well as seriously emotionally disturbed children and their families.
 
We are seeking a Care Coordinator who will provide quality care coordination and case management working with clients with serious mental illnesses while embracing, understanding and operating under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, and Thank You) and the spirit of motivational interviewing.
 
 A successful Care Coordinator will:
  • In collaboration with the MD/CNS and treatment team, formulates, develops and revises treatment plans, BPSA’s, ANSA/CANS assessments, and other necessary client assessments timely, accurately, and with good quality

  • Use discretion and independent judgment to develop treatment plans, in consultation with the treatment team
  • Advocate for clients to ensure access to needed services which includes developing community relationships and resources
  • Implement treatment plans, coordinates service revisions based on treatment plans
  • Identify any high risk patient symptoms or circumstances that would result in risk for self-harm or harm to others and notifies appropriate higher clinical level staff for consultation
  • Complete appropriate legal documentation as indicated on treatment plan
  • Interpret and applies service and billing procedures according to client needs; completes billing according to guidelines for timeliness and accuracy
  • Work with family, significant others and community supports to assist client in meeting goals
  • Alter activities/behaviors to reflect and ensure adequate care appropriate to the age of clients served
  • Work to fulfill duties noted in area specific competencies (Adult, Acute, Children’s or Addiction Services)
  • Meet the agency’s standard of timely completion of progress notes
  • Meet the agency’s standard of client service hours
  • Provide quality case management and activities of daily living skills training to clients
  • Monitor client progress and reflects this in documentation
  • Demonstrate commitment to professional growth by actively participating in clinical supervision
 
 The qualified candidate will:
 
  •  Possess a Bachelor’s degree in social work, substance abuse, education, counseling, psychology, sociology, criminal justice or related field, plus a minimum of one year of related work experience OR
  • Bachelor’s degree in a non-behavioral field of study with significant course work in behavioral science plus a case management certificate from a certified and accredited program with a professional school of behavioral science
  • May substitute a Master’s degree in social work, substance abuse, education, counseling, psychology, sociology, criminal justice or related field if no prior experience in mental health
  • Valid Indiana driver’s license that meets Eskenazi Health’s driving policy requirements
