Marketer/Community Outreach Representative for Home Care Agency - NYC, NY

Purpose: True Care's Community Outreach Representative will play a key role in expanding and developing list of community contacts and referral sources at MLTCs.

Responsibilities include:

  • Expand and develop list of community contacts and referral sources at MLTCs
  • Responsible for consistently developing and maintaining a list of MLTC referrals in different communities through outreach in nursing homes, senior centers, hospitals, agencies etc.
  • Reach out daily to contacts regarding possible case offers.
  • Understand the needs of each referral source at MLTCs to strategize and ensure that True Care can meet those needs.
  • Assist with new patient/case enrollment process for all MLTCs and creating an organized tracking system of all referrals.
  • Handling various Medicaid related issues from patients in a professional manner while staying in compliance

Qualifications:

  • College degree or equivalent experience preferred
  • Strong organizational skills; ability to keep track of new referrals and follow-ups as needed
  • Experience with office applications and healthcare software
  • Consistently exercise sound and responsible decisions, with the ability to make rapid and immediate decisions when necessary.
  • Excellent customer service, phone & people skills.
  • Ability to conduct presentations professionally and articulately in front of large groups
