Needs Assessment Counselor

Prairie St. John's, a 110-bed psychiatric care facility located in Fargo, North Dakota, has been providing services for children, adolescents and adults to address mental health issues, chemical dependency or addiction and co-occurring disorders since 1997. Prairie offers a full-service psychiatric and addiction Continuum of Care. This Continuum of Care includes inpatient hospitalization, partial hospitalization, residential treatment, intensive outpatient services and clinic services. Prairie St. John's currently has a Needs Assessment Counselor position available. The candidates selected will be responsible for the following:
 

  • Effectively handling phone calls and walk ins, collecting demographic and clinical information in order to triage for an assessment, clinic appointment or referral
  • Completing a thorough comprehensive needs assessment to determine appropriate level of care including obtaining and reviewing information from community providers
  • Is aware of safety and risk factors and situations and takes proactive steps to reduce risk
  • Effectively and concisely communicating assessment results with physicians, referents, patients and families, and staff regarding findings and disposition
  • The individual will be scheduled to work 32 hours per week on the PM shift (3 – 11:30 pm) and one weekend per month (Friday, Saturday, Sunday).
Job Requirements
  • Knowledge gained through completion of a Master’s degree in Social Work, Counseling, Nursing, or a closely related therapeutic field, or a combination of closely related work experience and or related training.
  • Qualified candidates may have experience as a Registered Nurse in a behavioral health setting in lieu of the Master’s level education.
  • Must meet the requirements of Mental Health Professional as defined by NDCC 25.03.1. Licensure as a Licensed Certified Social Worker (LCSW) or as a Licensed Professional Counselor (LPC) or as an Advanced Practice Nurse Practitioner (RNCS) is preferred, or licensed as a Registered Nurse with extensive experience in Psychiatric and chemical dependency conditions.
  • Experience using Microsoft Office Products including Word and Outlook.
