About this Career Opportunity:
- The Registered Nurse (RN) is responsible for providing care and treatment for clients in their home environment or in a heathcare environment setting.
- Provide 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence.
- Directing and supervising the activities of the licensed practical/vocational nurses, CNAs, and caregivers as needed.
- Monitoring clients vital statistics and reporting abnormal findings to the case manager and physician
- Administration of medications, feedings, oxygen, ostomy care as needed per client's plan of care
- Complete routines as established by other care professionals such as PT, OT, Speech Therapists
- Ensure infection control policies are maintained at all times
- Follow client's plan of care.
- Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians and documentation of progress and outcomes for established goals
- Informs physician, case manager, and families of changes in client's medical condition and needs
- Ordering supplies, medications, and feedings as needed to insure adequate amounts are on hand at all times
- Establishing and maintaining a professional relationship with families, clients, and other health providers
- Report incidents or accidents to case manager
- Ensuring all medical equipment is maintained and report faulty equipment to DME for repair
- Performs other duties as assigned
