Advanced Behavioral Health Clinician

Responsibilities
Working within an interdisciplinary care team thoughtfully orchestrated to support all aspects of members' needs, the Advanced Behavioral Health Clinician is responsible for triaging cases referred for behavioral health and psychosocial support and implementing clinically indicated interventions. 
Consistent with existing team-based care models, the Advanced Behavioral Health Clinician collaborates with all members of the Edge Clinical Team including Navigators, Registered Nurses Pharmacists, Psychiatrists, Primary Care Physicians and Community Health Workers to plan and implement optical care for medically and psychosocially complex members. This includes both direct provision of care as well as referral to appropriate internal and external counterparts to facilitate the necessary services to support members' overall health and well-being. 
This position requires a comprehensive understanding of behavioral health diagnoses as well as chronic medical conditions, and how the two interrelate and affect one's global health. Experience should include crisis management, psychosocial assessment, intake and triage decision making regarding levels of care and time-limited counseling. Knowledge of local community-based resources for behavioral health care is also needed. 
As this is a newly developed care team, adaptability, flexibility and willingness to participate in process and workflow improvement and serve as a clinical leader and source of support to other clinicians brought on to the team is vital to success in this role. 
Major Duties and Responsibilities
  • Support the implementation and ongoing integration of behavioral health resources and supports available to Edge members
  • Collaborate closely with the Edge's Virtual Psychiatrist to coordinate behavioral health/SUD services provided to members
  • Upon receiving a referral, the Advanced Behavioral Health Clinician will perform a complete psychosocial assessment of the member's symptoms, psychiatric treatment history, contributing medical conditions, social supports, caregiver concerns, barriers to accessing care including transportation or potential stigma/bias against receiving care. The clinician will then collaborate with members of the care team to prepare an intervention plan
  • Provide urgent telephonic safety assessments for members calling into the plan in crisis, and initiate intervention as needed
  • Participate in integrated mental health case reviews for complex members
  • Serve as the source of knowledge on behavioral health diagnoses and recommended evidence-based treatments to primary care colleagues
  • Employ a range of time-limited and focused interventions such as individual, family or group counseling. These interventions may be carried out virtually through Telehealth visits, in a member's home or in a community-based Edge operated space for members
  • Complete referrals for behavioral health care as indicated to other team members such as the Virtual Psychiatrist for virtual medication consultation, or to community-based providers
  • File reports as mandated for suspected abuse and/or neglect of children, disabled persons or the elderly
  • Work closely with the Community Health Workers on the team, eliciting their assistance with barriers to wellness such as lack of transportation or food insecurity
  • Serve as the clinical leader in a group of six other Behavioral Health Clinicians, providing direct support to clinicians on site, and conferring with the Associate Director of Behavioral Health for assistance when needed
  • Act in accordance with the NASW Code of Ethics
  • Perform other duties as required

  • Demonstrate a desire and commitment to meeting members' needs as well as improving the overall experience of interactions with the Edge
  • Take initiative to creatively confront workflow inefficiencies and collaborate with the team to improve processes
  • Participate in supervision and continuing education to maintain Licensure
  • Document patient encounters in medical record system in a timely manner
  • Follow general policies related to fire safety, infection control and attendance
Qualifications
  • Master's Degree in Social Work from an accredited institution
  • LISW-CP Licensure
  • Over five years of experience working in the mental health field preferred
  • Experience working in medical social work or with the geriatric population preferred

Working Conditions
This role will be centrally located with the other members of the multi-disciplinary team, but will at times involve travel to members' homes and community-based settings to facilitate connection and deliver direct care. Hours will retain some flexibility to meet members' needs but will follow a Monday-Friday daytime schedule.
