Integrated Health Services Enrollment Specialist

Through care coordination, YSS's Integrated Health Services (IHS) program connects families with resources to help their children be successful at home, school, and in the community. Families are partnered with a team of professionals to ensure their child is receiving the necessary care for their physical, behavioral, and mental health needs.

The role:

As an Integrated Health Services (IHS) Enrollment Specialist, you will play an active role in designing an integrated care plan for children and their families. As the first point of contact for clients, you will be responsible for completing assessments, making and tracking referrals, following up on referrals, and coordinating with internal team members and other professionals.

What you will need:

  • A Bachelor’s degree in social work or a related human services field is required.
  • Knowledge and work experience with mental health and child welfare is required. Work experience may be substituted for the educational component.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

What you will do:

  • Assist with the client enrollment process.
  • Use available data, such as assessments, participant evaluations, and service observations to identify health and wellness needs to develop individualized care coordination plans.
  • Educate clients and their families or care advocates in wellness, medication, and behavioral health services.
  • Develop crisis plans as necessary.
  • Provide transportation to appointments when necessary.
  • Coordinate and be familiar with all aspects of outpatient funding, including an understanding of EMR and funding coordination internally as well as credentialing and compliance with third-party payers.
  • Schedule and coordinate initial, crisis, intake, and other appointments with clients.
  • Demonstrate flexibility in job duties and schedule in order to serve clients and to assist reaching benchmark standards.
  • Participate in ongoing quality improvement efforts and monitor/evaluate service outcomes.
  • Establish positive relationships and networks with community partners on a routine basis.
  • Provide community presentations on relevant topics and is accessible to the media.
  • Other duties as assigned.

Compensation:

$32,000/year
