Transfer Center of Eastern Maine where "One CallStarts it All" for referring facilities and providers requesting consultation, access to services or transfer of acute patients to EMMC.
The Transfer CenterNurse consists of two distinct nursing roles divided between coordination of transfers into EMMC and internal patient flow and bed placement. The Transfer Center Nurserotates responsibilities between these two job functions.
In the Transfer Coordination role, the Transfer Center Nursewill be the single point of contact for facilities/providers requesting access to services or patient transfer to EMMC. The Transfer Center Nurseobtains patient information critical to determining the appropriate EMMC provider, medical necessity and level of care; coordinates the phone call between the referring and receiving providers and assists referring facilities in selecting appropriate mode of transportation.
In the patient placement role, the Transfer Center Nurse will be responsible for facilitating patient flow within EMMC, balancing bed requests with priority of need.
Weekend and Holiday rotation required.
