SUMMARY
The purpose of the Case Manager position is to ensure a child's safety and movement toward permanency using legal and administrative processes as prescribed by the Department of Children and Family Services and the courts.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Provide services to an average case load of 12-15 children.

Work with families to achieve permanency through adoption, subsidized guardianship, or return home, or otherwise achieve independence or other alternate options deemed necessary.

Assure consistency of efforts among children, foster parents and natural parents

Conduct monthly home visits to foster homes to monitor client’s safety, well-being and service needs.

Attends all court hearings, Administrative Case Reviews, CIPP, Individualized Educational Meetings, etc. as required.

Transports clients in personal vehicle as needed.

Produce all required documentation in an accurate and timely manner

Complete all Core Curriculum training(s) required for the position.

Perform all other duties as assigned.


QUALIFICATIONS 
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Must complete Illinois State Child Welfare Licensure


EDUCATION and/or EXPERIENCE
Bachelor's degree in Social Work or human services related field required

LANGUAGE SKILLS
Ability to read and interpret documents such as case notes, treatment plans and procedure manuals. Ability to write routine reports, case notes and correspondence. Ability to speak effectively before groups of clients or employees of organization.

REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

PERFORMANCE IMPROVEMENT
Ability to understand and demonstrate Aunt Martha's 5-step Performance Improvement process, utilizing the methodology PDCA (Plan, Do, Check, Act).

JOB KNOWLEDGE
Ability to use technical skills and apply the knowledge necessary for this position.

QUALITY 
Ability to demonstrate accuracy and thoroughness while performing the duties of this position. 

SAFETY AND INFECTION CONTRO
Be knowledgeable about and follow Aunt Martha's safety policies and procedures.

OTHER QUALIFICATIONS
AMYSC employee shall obtain and maintain certification in First Aid and CPR. Certification must be obtained within 3 months of employment.
Current valid Illinois driver’s license
Documentation of current valid auto insurance
Clear driving record
Ability to work on after hours, holidays and weekends
Ability to carry an agency provided cell phone 24 hours a day, to respond, when needed for crisis or regular job responsibilities.
Ability to transport clients in personal vehicle that is clean and smoke free


CERTIFICATES, LICENSES, REGISTRATIONS
DCFS Child Welfare Licensure and Child Endangerment Risk Assessment Protocol (CERAP) certification

OTHER QUALIFICATIONS
AMYSC employee shall obtain and maintain certification in first Aid and CPR. Certifications must be obtained within 3 months of employment.

Ability to carry an agency provided cell phone 24 hours a day, being available when needed for crisis or regular job responsibilities.
Ability to transport clients in reliable personal vehicle that is clean and smoke free
