Case Manager-First Shift

Case Manager provides case management services to adults and their families. It is designed to serve clients who have a severe and persistent mental illness and need linkages and supports to remain stable and independent in the community. The Case Manager will provide services deemed medically necessary, case management, treatment planning, assessment etc.

• Work hours prescribed and any additional hours deemed necessary for the responsibilities assigned.
• Develop and complete required assessments within time frame, develop and implement interim and long term treatment plan with client, coordinate psychiatric services and treatment along with psychiatrist and nurse
• Coordinate and collaborate with all assigned team members, psychiatrist and nurse regarding comprehensive services provided to participants, including complete information sharing and coordination of delivery of services
• Provide referral and linage to additional internal and/or external services based on need or presenting severity of symptoms
• Graduate clients from services whenever possible by lining them to their primary care providers for continued prescribing needs
• Consult with LPHA’s, psychiatrists and nurses for disposition
• Have full knowledge of various programs at Rosecrance and the referral process
• Provide case management services and all other medically necessary services
• Coordinate immediate treatment and complete appropriate referrals for intervention when needed
SHIFT: Mon-Friday 8 am - 5 pm

Position Requirements
• Bachelor’s Degree in counseling, psychology, social work or another related field
• Knowledge and experience with psychopharmacology preferred
• The ability to communicate clearly verbally and write concisely and legibly
• Ability to make recommendations and linkages based on reported side effects, symptoms and/or concerns
• Ability to assess mental status based on reported and observed symptoms
• Ability to quickly establish rapport with others
• Ability to perform in an ethical manner
• Ability to follow written and oral instruction
• Valid driver’s license
