Registered Nurse Triage Coordinator - Direct Access Center

The Direct Access RN Coordinator is the key link between Meriter and referring physicians and hospitals. S/He obtains clinical information and physician orders from referral sources, determines appropriate level of care, assigns patient status (admission, observation or outpatient), and coordinates the appropriate placement of patients requiring admission, transfer, or for clinically complex outpatient services. Safe and appropriate placement of patients maximizes census and enhances fiscal accountability. As part of the coordinated patient placement strategy the Coordinator will track and monitor pending discharges and observation status. The Direct Access RN Coordinator acts as a resource and liaison between Emergency Room, Nursing Units, Physician's Offices, Urgent Care, outlying hospitals and other sources of patient admissions and inpatient units. As the clinical needs and patient levels change throughout the shift, the Coordinator will evaluate staffing levels on inpatient units and communicate the assessment to the NAC and/or appropriate administrative staff. 

Qualifications
  • Graduate of a Bachelors of Science in Nursing (BSN). Will consider other nursing degrees on an individual basis
  • Registered Nurse in the state of Wisconsin
  • Three years current nursing experience. Proven leadership experiences as unit charge nurse
  • Proven leadership skills in inter and intra departmental settings
  • Proven expertise in understanding acute care criteria from inpatient hospitalization, detailed knowledge of scope of practice of each nursing unit and a working knowledge of its operations (or a similar setting)

  • Previous management experience or team leadership experience
