Community Wellness Coordinator
Responsible for coordinating efforts and resources to meet the needs identified in the tri-annual Community Health Needs Assessment (CHNA). Collaborate with regional partners to stay abreast of identified health disparities, evidence-based practices, and trending data. Collaborate and meet with key stakeholders within the organization and local schools at a minimum of semi-annually to identify health and wellness needs. Coordinate and oversee health-related grants awarded to GBAHC as assigned ensuring objectives and outcomes are achieved. Build and foster positive external relations with local businesses, schools, and civic groups. Facilitate meetings related to the development, planning, and implementation of outreach efforts. Serve as a liaison between the health system, organization, and community to enrich every life we touch. Collaborates and communicates with Director of Community Relations and Director of Rehab Services to identify and develop educational material, programming, and campaigns related to health and wellness topics.
