Social Worker/Social Worker, Senior

THE POSITION
The Hennepin County Health and Human Services Community Integration and Recovery Team (CIRT) in the Behavioral Health Case Management (BHCM) area is seeking a Social Worker or Senior Social Worker. The best qualified candidate for this position will have a licensure in social work and be a current Licensed Alcohol and Drug Counselor (LADC) in the state of Minnesota.

CIRT provides interd­isciplinary case management services for adults with serious persistent mental illness and/or substance use disorders. A person-centered, recovery approach is the model utilized in Behavioral Health. Behavioral Health case managers must meet the requirements for mental health practitioner or mental health professional in MN Statutes 245.4871.

Social Worker $57,225.27 - $71,247.95 Annually 
Senior Social Worker $58,179.28 - $83,801.71 Annually 

Location and hours:
This position is currently being performed remotely due to COVID-19. Work hours are primarily days, Monday through Friday. No weekend, overnight or holiday shifts are required. Once working on-site, this position is located at the Behavioral Health Wellness Center, 1800 Chicago Avenue South, Minneapolis, MN 55404.

About the position type:
This is a full-time, benefit-earning position.

In this position you will:
  • Provide mental health targeted case management to individuals civilly committed due to mental illness, or mental illness and substance use.
  • Engage individuals in treatment plans incorporating their strengths, resources, and goals to ensure health, safety and access.
  • Assess the needs with a person-centered and culturally competent approach to determine a plan of action derived from Motivational Interviewing and stage based intervention. 
  • Provide supportive counseling to family members and concerned persons.
  • Work with attorneys (both criminal and civil) and the court on civil commitment matters and criminal proceedings. This may include the Rule 20 competency process. 
  • Monitor and manage court related activities. 
  • Complete comprehensive assessment to address treatment for substance use services.
  • Facilitate substance use referrals and care coordination.
  • Complete and assist with updating Rule 25.
  • Work collaboratively and effectively with the interdisciplinary team and outside providers. 
  • Document efforts in a timely and thorough manner, meeting or exceeding the program’s productivity standards. 
Need to have:
  • For hire as a Social Workeroneof the following:
  • Bachelor's degree in social work, psychology, sociology, human services, or equivalent field
  • Licensed as a Social Worker by the Minnesota Board of Social Work.
  • For hire as a Senior Social Workeroneof the following:
  • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare.
  • Bachelor's degree in social work, psychology, sociology, or human services, and at least 27 quarter credits of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker.
  • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work.
  • A valid driver's license and daily access to reliable transportation to travel primarily within the metro area to meet with persons in their residential settings and/or perform authorized off-site work duties. 
Nice to have:
  • Current Licensed Alcohol and Drug Counselor in the state of Minnesota.
  • Knowledge of cultural differences in beliefs and practices relating to mental health treatment, family structure and dynamics.
  • Experience:
  • Working with case management servicing people with substance use and co-occurring disorders. Interviewing, collecting, and analyzing information to reduce risks and increase safety.
  • Coordinating with case management within diverse communities to create safety plans.
  • Working in protective services of children and/or vulnerable adults.
  • Working and managing crisis situations.
  • Working with the Probate Court system.
  • Knowledge of community resources, health care facilities, in-home services, medical and psychiatric disorders, and risk assessment and management.
