Independent Living Specialist

Job Title:            Independent Living Specialist
Reports to:        Director of Medicaid Programs
FLSA Status:      Non-Exempt                                                              
 
Job Summary: Provides specialized instruction to individuals receiving Supported Community Living services.
 
Essential Functions: Identifies main accountabilities performed in the job.
 
I.                Provides instruction to individuals receiving Premier Payee services.
·       Medication support (we do not administer, store, or transport medications)
·       Nutrition
·       Budgeting and money management
·       Transportation coordination
·       Assistance with entitlements
·       Crisis Management and problem solving
·       Home management skills
·       Socialization and leisure/community involvement
·       Safety planning
·       Development of natural support systems
·       Educational skills development/planning
·       Obtaining and retaining housing
·       Advocacy for clients and their care & Personal Hygiene
 
II.               Oversees and assists with consumer’s needs.
·       Develops of a comprehensive assessment of each consumer’s needs;
·       Assists as assigned by the Director of Medicaid Programs, writes, and implements
individualized curriculums and interventions as described in the IPP;
·       Plans, teaches, and assists with recreational and leisure-time activities;
·       Teaches informal activities to supplement formal activities and training;
·       Employs crisis intervention techniques including verbal de-escalation and physical intervention as required.
·       Interacts with individuals in services in a friendly, respectful manner.
·       Acts as an advocate in a professional manner for those in services.
·       Teaches personal hygiene skills as determined by the individual’s level of need.
·       Administers medication as prescribed by the physician and in accordance with Premier Payee policies and procedures.
·       Ensures proper control of medication.
·       Ensures the health, safety and well-being of those receiving services in accordance with Premier Payee policies and procedures.
·       Assists with the development of the Individual Program/Treatment Plan.
·       Maintains all applicable records.
·       Provides pertinent information regarding consumer needs, activities, and interests.
·       Maintains positive, professional relationships with consumers, guardians, service coordination, families, supervisors, co-workers, and professionals from other agencies.
·       Protects the confidentiality & rights of all individuals served and employed by the agency. Promotes a positive image of the agency and those individuals it serves.
·       Attends all trainings as required by the agency.
·       Performs other duties as necessary to meet the needs of Premier Payee.

General Requirements:  
Minimum of high school diploma is required with two years’ experience interacting with people with behavioral issues, mental health needs, or developmental disabilities. Extensive practical experience in working with a difficult client population may be substituted for education. Desired qualifications are a bachelor’s degree in human service-related field and must be at least 19 years of age. Must possess a valid driver’s license, valid/current automobile insurance, and reliable transportation.
 
Knowledge, Skills and Abilities:  
Demonstrates an active understanding and proficiency with calculations. Displays strong interpersonal verbal and written skills with ability to prioritize responsibilities and adapt to changes in daily tasks when necessary.  Must be flexible and accommodating to go above and beyond the basic expectations when required. Must be detailed-oriented. Ability to work independently with a minimum of supervision while coordinating activities with several departments. Ability to work with people in a courteous and professional manner on the phone and in person. Must be able to maintain composure in emergency situations. Must be willing to learn and physically capable of implementing crisis intervention techniques.
