Medication Reconciliation - Student Pharmacy Technician

Seeking a student pharmacy technician currently enrolled in the College of Pharmacy
 
Schedule: Shifts include one 4-hour shift during the week and two 8-hour shifts every other weekend
 
Position Description
Under general pharmacist supervision, coordinates and performs medication histories throughout the institution. Additionally, the student pharmacist might be assigned clinical tasks, dispensing tasks (medication messages), and other tasks as requested by the supervising pharmacist. The student will report to the inpatient clinical pharmacy mangers
 
Position Responsibilities Include:
  • Obtain and clarify patient allergies and prior to admission medication histories from relevant sources including but not limited to: patient interview, phone call to outpatient pharmacy/nursing home/provider’s office, outside facility records etc. Such information should be processed and documented in the Electronic Health Record (EHR)
  • Directly communicate to the pharmacist about the patient with complex/high‐risk medications inclusive of prior to admission medication list as defined by policy
  • Document any appropriate medication history related intervention in EHR
  • Facilitate or direct appropriate medication related questionnaires to the pharmacist
  • Communicate effectively with other members of the health care team on a regular basis
  •  Within scope of practice assist pharmacist to facilitate pharmaceutical care to the patients
  • Assist with on-the-job training of pharmacy students and new technicians
  • Review documentation records for completeness and contact manager if documentation is not complete
 
Minimum Eligibility Requirements
  • Currently enrolled as a P1-P4 student at University of Iowa College of Pharmacy
 
Special Qualifications
  • Ability to calculate ratios & proportions for pharmaceutical needs as demonstrated by a work sample
  • Ability to know the brand/generics of the most commonly seen medications as demonstrated by a quiz
  • Ability to know the indications of the most commonly seen medications as demonstrated by a quiz
