This position oversees Human Services-Information and Referral, PA 211, and the Link Services Access Program. This position will provide knowledgeable directions, suggestions, and information to any individual seeking assistance for potential available Social Determinant of Health (SDOH) human service programs, functions, and agencies. The Information Access and Link Services Coordinator will also begin the preliminary first step linking to these services for individuals in need. As the all-access point of contact for Franklin County, this position will also be directly responsible for the formation and oversight of a Training Day committee to assist with the creation and scheduling of the Human Services Professional Development Day(s).

 

The LINK Services (ADRC) coordination responsibilities are to create a seamless approach in the way assistance and resources referrals are given to seniors and adults with disabilities in the community. These individuals may need assistance with activities of daily living or connecting with medical insurance and other resources and referral needs. The ADRC is better known as the Aging and Disability Resource Center within PA.  

 

Requirements include: Minimal Associate’s degree in human services, social services, or behavioral sciences field is required. AIRS certification is a plus; not necessary as training will be provided. Bilingual English/Spanish speaking skills are a plus, as well as knowledge of sign language, or other interpreter skills for communication purposes. Minimum of two (2) to four (4) years of human service experience and knowledge of human services; and computer experience in word processing and database required. Consideration will be given for any equivalent combination of education, experience and training.
