Integrated Health Home (IHH) Supervisor

First Resources Corp. of Mt. Pleasant is growing and we are searching for dedicated and energetic Full-time Integrated Health Home (IHH) Supervisor. The ideal candidate would be responsible for coordination and growth of Integrated Health Home services. Promote wellness, resiliency, and recovery by providing stength-based person-centered care coordination services to adult individuals living with mental illness.

An Integrated Health Home (IHH) Supervisor is there to:
  • Schedule and facilitate IHH team meetings including huddles, staff meetings, and QI meetings.
  • Schedule and oversee the IHH team in attending required MCO/IME trainings and webinars.
  • Oversite of the IHH performance measures.
  • Oversite of the IHH referral process.
  • Oversite of the IHH master tracking.
  • Be a liaison between FRC and IME, MCO, DHS, community partners, families, members, etc.
  • Partner with the fianance team on IHH billing when needed, helping to resolve any concerns that arise.
  • Oversite and utilization of the Medicaid portals including but not limited to: IPMA, IoWANs, EDISS, ELVs, Evolve, Availity, etc. This includes ensuring the IHH team is utilizing the portals to the best of their ability.
  • Promote wellness, resilency, and recovery by providing stength-based person-centered care coordination to members.
  • Linkage and support for community resources, insurance assistnace, waiver services, peer advocacy groups, family support networks, wellness centers, NAMI, family psychoeducational programs, support groups, etc.
  • Provide comprehensive transitional care, including education on preventing unnecessary emergency room use
  • Linkage to transportation services or transport members to important appointments
  • Linkage to housing resources or shelters
  • Monitor and advocate for member individual rights
  • Encourage member engagement in their goal development and achievement
  • Provide caseload members with at least one IHH Core Service per month
  • Conduct level of care assessments, functional assessments, and social histories
  • Make referrals on behalf of the member and conduct continuous monitoring of the referral outcome.
  • Monitor for hospitalizations, ER visits, and follow-up appointments Work hard on reducing ER and hospitalizations and increasing 7-day follow up appointments when appropriate
  • Oversite of members' status on achieving healthcare goals and provide member support and encouragement on achieving those goals, including IHH measure standards when appropriate
  • Participate in the on-call rotation to be available to IHH members served for consultation.

Requirements:
  • Bachelor's degree with at least 30 semester hours or equivalent hours in a human services field (including, but not limited to, psychology, social work, mental health counseling, nursing, education, occupational therapy) and at least one year of experience in the delivery of services to individuals diagnosed with a mental illness.
  • Or
  • An Iowa license to practice as a registered nurse (RN) and at least three years of experience in the delivery of services to individuals diagnosed with a mental illness.
  • Previous supervisory experience preferred.
  • Must have valid driver's license and personal auto insurance as required by the State of Iowa and be insurable under the company's insurance.
  • Must be willing to use personal vehicle when needed for work-related events and must be willing to transport members in personal or company vehicle when needed.
  • Must have access to a telephone.
  • Ability to travel at least 30% of the time.
  • Must possess basic knowledge of computers and computer-based programs.
  • Must have a valid driver's license, a good driving record and show proof of auto insurance.
  • Satisfactory results on background check pertaining to criminal and abuse registries.
