Mental Health Practitioner II - SFT Nexus-FACTS

Nexus-FACTS Family Healing, an outpatient/community mental health clinic, has an exciting opportunity for a full-time Mental Health Practitioner II position for our Systemic Family Therapy (SFT) program based in our Mendota Heights office.

Responsibilities:
The SFT therapist will provide therapy services to at risk youth and their families. These services are primarily in-home and in-community. SFT is a generalist model that is designed to engage and treat the multiple systems that are part of an at-risk youth’s ecology.
  • Conduct individual, family, and marital therapy according to the SFT model.
  • Develop diagnostic assessments and treatment plans that identify strengths, goals and objectives.
  • Coordinate case with identified providers, attend to managing the client file, take into account all aspects of the client needs (psychosocial, financial, spiritual, etc.).
  • Implementation and administration of policies, programs and activities

Qualifications:
  • Minimum of a Master's degree in Marriage and Family Therapy, Social Work, Human Services, or Psychology
  • Prefer candidates who possess or are on track to complete licensure for a specific related discipline (example: LGSW, LAMFT, or LPC. LICSW, LMFT, or LPCC)
  • Prefer at least 1-2 years providing children, adolescents, and family-based behavioral health services
  • Valid driver's license required. Must meet state regulating agency driving requirements and pass corporate insurance eligibility standards
