Social Worker, Senior Psychiatric

The Human Services and Public Health department (HSPH) is seeking a Senior Psychiatric Social Worker who has an interest in providing outreach and case management to individuals experiencing homelessness, and working as part of Hennepin County’s Health Care for the Homeless multidisciplinary team. This position requires the ability to work independently as a Senior Psychiatric Social Worker and as part of a larger team to provide wrap around services to unsheltered clients living with HIV, as part of Public Health’s Drug Related Infectious Disease (DRID) response. The position will provide outreach to individuals experiencing homelessness in Hennepin County, help support others on the team to offer access to programming and services as part of a person-centered approach, and carry a caseload of clients. Location and hours:
This position is hybrid and performed in the community, on-site at the Health Services Building located at 525 Portland Avenue, Minneapolis, Minnesota 55415, and remote as job duties require. Typical hours are 8 a.m. - 4:30 p.m. Monday through Friday with one or two evenings per week. No weekend, overnight or holiday shifts are required. Orientation and training will be performed on-site.

While this position is designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin. 
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will: 
  • Engage, establish trust and build rapport with unsheltered individuals living with HIV.
  • Establish appropriate cares: HIV specific clinic/treatment, Primary Care, Mental Health, Substance Use Disorders.
  • Provide mental health support/care including psychotherapy and group therapy.
  • Ensuring basic needs are met, assistance with resources and referral.
  • Complete the Coordinated Entry Assessment and assist clients with housing. 
  • Assist with transition planning when client moves into permanent housing: Ryan White Medical Case Management, housing stabilization services, MH Case management, etc.
  • Participate in the Healthcare for the Homeless' quality improvement efforts, including those directed toward anti-racism, equity, and trauma awareness.
  • Follow-up with clients to ensure smooth transition into housing.
