Integrated Health Home Program Coordinator

SUMMARY DESCRIPTION OF WORK

Supervisory responsibilities and major agency wide support functions begin at this level.  Decision making, problem solving, and ongoing task prioritization are critical.  Training in supervision and management is required.

The IHH Program Coordinator will be responsible for supervising IHH staff, problem-solving client issues, initializing client engagement in the IHH program, and managing client assessment needs.  They will serve as the primary enrollment contact, be responsible for monitoring engagement needs of newly enrolled members and ongoing assessment and authorization needs of high need, high risk clients. Additionally, this staff will assist with program data entry, monthly and annual reporting of data, and maintaining data quality standards. This position requires travel throughout southwest Iowa. 

Compensation range: $49,200-$62,243 based upon experience.
Work Schedule: Monday through Friday, 40 hour work week

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree; and 5 years’ experience in related field (Integrated Health Home experience preferred)
  • Supervisory/program management experience is a plus
  • Prefer proficiency in MS Word and Excel
  • Valid driver’s license/acceptable driving record

Essential Duties and Responsibilities

  • Provides positive leadership and promotes the trauma informed culture for all staff, program clients and in the community
  • Provide overall coordination of IHH staff, ensuring daily staff coverage and responsibilities are completed as required.
  • Assist in training, monitoring, evaluating, and developing program staff
  • Is able to develop positive working relationships with clients, referral sources, service agencies and others encountered in the course of work.
  • Lead weekly team meetings, including approving enrollments, reviewing caseloads, and providing feedback on case plans and service delivery.
  • Assist with conducting case management duties as part of the IHH team
  • Understand, monitor, and provide process oversight to high need clients in habilitation services.
  • Collaborates with Program Director to improve policies and procedures to ensure compliance
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency
  • Abides by all specific program and Agency procedures, policies, and requirements
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff
  • Is able to develop positive working relationships with clients, referral sources, service agencies, landlords, and others encountered in the course of work.
  • Essential functions of this job are to be performed on company physical work site.
  • Performs other program related duties as assigned.
