PAY AND BENEFITS

Annual Pay Range: $53,767.31 - $67,749.92

Hourly Pay Range: $27.572979 - $34.743549

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Social Services Division is currently seeking a Case Manager to provide a strengths-based, person-centered approach to provide low-barrier supportive services for those who might not meet the federal definition of veteran, and veterans and their families who are experiencing homelessness or are at risk for homelessness. This includes eligibility determination, obtaining and retaining safe and affordable housing in Clackamas County and partnering with a wide variety of community resources. All services will be delivered in a manner that is culturally responsive including military culture.

The Case Manager will also help individuals access other needed support and services to include increasing income, enhancing physical and behavioral health, accessing veterans benefits, and other personalized assistance based on individual needs. Required Minimum Qualifications/ Transferrable Skills:*

• A minimum of two (2) years of relevant experience that would provide the required knowledge and skills to perform the duties of the role
• Direct case management experience or a comparable function, working with homeless and/or low income populations
• At least one (1) year using creative problem solving, with successful results, in preventing people at high risk of homelessness from becoming homeless or helping people who have been homeless access and maintain permanent housing
• Knowledge of State and Federal laws, rules and regulations applicable to government programs and funding guidelines, including the HUD HEARTH Act, Oregon EHA and other similar programs
• Experience using Microsoft Office products including Outlook, Excel, and Word
• Experience using virtual programs such as Zoom and Microsoft Teams
• Knowledge of methods and techniques of strengths-based case management, principles and techniques of interviewing, offering supportive services and crisis intervention
• Knowledge of effective recordkeeping techniques and skill to maintain accurate and concise records
• Ability to work effectively in a multicultural environment and to advance equity in all aspects of program delivery

Preferred Special Qualifications/ Transferrable Skills:*

• In-depth knowledge, demonstrated by veteran status, education, training and/or working experience, of veteran needs and advocacy
• Knowledge of social, health care, culturally specific, financial, and general community resources
• Bilingual fluency in English and Spanish

TYPICAL TASKS

Duties may include but are not limited to the following:

• Assess and document program eligibility. Accept referrals from Coordinated Housing Access system. Gather documentation and information to confirm initial program eligibility and recertify annually as required by funding source.
• Provide housing placement and housing retention services using a person centered, housing first model to approximately 20 homeless or formerly homeless individual and family veteran households who meet HUD's chronic homelessness eligibility. Utilize language interpreter service as needed to ensure equal access to housing and support services.
• Promote and advance equity in approach to work in case management services, initial and ongoing engagement, with teammates, social services staff and in broader community as appropriate.
• Establish participant rent payment levels based on participant income, total unit rent and federal, state and grant requirements. Prepare initial and ongoing rent and related payment vouchers in compliance with applicable regulations.
• Collaborate closely with participants on person-centered action plans to access necessary services and achieve housing stability goals, including household budgeting. Modify action plans as required to align with client needs, preferences, and values, utilizing assessment and communication skills. Facilitate access to additional desired services using knowledge of both public and private resources.
• Collaborate with healthcare providers, social service personnel, county veteran service offices, VA, and other agencies to secure necessary services. Regularly reassess and update case plans and client eligibility criteria. Advocate on behalf of participants with public and private agencies, schools, and other entities.
• Maintain participant records in a timely, complete and organized manner and prepared for audits and reviews as necessary.
• Participate in a rotation with other supportive housing team members to provide backup for direct service staff in case of unplanned or scheduled absences in various programs.
• Gather and submit necessary data for the Service Point Homeless Management Information System during entry, exit, annual recertification, and interim updates as required.
• Proactively communicate to prevent emergencies, including maintaining regular contact with landlords and property managers. Respond promptly to emergencies and negotiate solutions to resolve issues as they arise.
• Report witnessed or suspected abuse and neglect of children, older adults and/or people with disabilities.
• Other duties as assigned from Department of Health, Housing and Human Services leadership.

REQUIRED KNOWLEDGE AND SKILLS

Working knowledge of: Methods and techniques of strengths based case management, offering supportive services and crisis intervention; social safety net, health care, financial, veteran specific resources, and other community resources and agencies; effective record keeping procedures and techniques and skill to maintain accurate and concise records; State and Federal laws, rules and regulations applicable to government programs and funding guidelines, including the HUD HEARTH Act, Oregon EHA and other similar programs.

Skill and ability to: Work effectively in a multicultural environment and to advance equity in all aspects of program delivery; gather information through research and interviews with applicants, participants, families, caregivers and other relevant individuals; proficiently use Microsoft Office applications such as Word, Excel, and Outlook; proficiently use virtual platforms such as Zoom and Microsoft Teams.

WORK SCHEDULE

This position is included in the County's alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off):

• Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
